Microsoft is warning admins about Exchange Server instability after the October 2026 Patch Tuesday cycle. Since October 13, some on-premises servers have started failing under load. However, the culprit is not an Exchange patch. Instead, the problem comes from the October 2026 Windows updates.
What Admins Are Seeing
The symptoms vary. For example, OWA may fail with the error “MapiExceptionMdbOffline: Unable to make connection to the server.” Event ID 1006 then shows up in the Application Log. In addition, mail queues can grow during the day until delivery crawls to a halt. Some DAG members also fail to reach quorum.
A reboot may help, but only for a while. Exchange Server 2016, 2019, and Subscription Edition can all be affected.
The Root Cause
According to Microsoft’s Exchange Team advisory, the trouble follows four Windows Server updates: KB5126646, KB5126655, KB5126651, and KB5126868. Microsoft stresses that “This is not a ‘known issue’ related to October 2026 Exchange update.”
The Windows Server 2025 release health page explains why. The update “introduced a default limit on the amount of memory used by Endpoint Mapper.” As a result, the default cap of 512 simultaneous lookup calls “might not be sufficient under heavy load.” That is why Exchange Server instability hits busy servers hardest. Some servers might never see a problem.
How to Fix It
First, do not uninstall the October Windows updates. Next, run this command from an elevated Command Prompt:
reg add "HKLM\SOFTWARE\Microsoft\Rpc" /v EpmapSavedContextCap /t REG_DWORD /d 65536 /f
Then reboot the server and keep watching. If the issue returns, raise the limit further. Microsoft also offers an extra mitigation through Microsoft Support for business.
Overall, the fix is simple, and it keeps the security patches in place. Admins who run busy mail servers should apply it now, before Exchange Server instability disrupts mail flow for users.
