On Apple’s official forums, users have already submitted feedback describing the issue: the upgrade process proceeds smoothly until the very final stage, where an error appears. The Mac then reboots and reverts to version 15.7.
The root cause appears to lie in a flaw with Apple’s Neural Engine driver. During installation, when the setup attempts to load the new Tahoe kernel driver for the Neural Engine, a hardware check fails, triggering the rollback.
Apple has begun contacting affected users to collect kernel crash logs in order to investigate the underlying cause. Analysis and the release of a fix may take some time, so M3 Ultra Mac Studio users are advised to wait for an updated version before attempting the upgrade.
